A guess about the extra purl stitch at the beginning of the needle: I'd be willing to bet that in moving the yarn from back to front at the transition between needles you somehow inadvertently created a YO - it's all too easy to do. I'll post a picture later this evening. But I did manage to get my hand through it, so the ribbing is stretching enough.
